Sensitivity that begins in middle age generally has an identifiable cause, and the important first step is distinguishing it from something that needs urgent attention.
The mechanism
Dentine is traversed by microscopic tubules communicating with the nerve. Where the surface becomes exposed, temperature and pressure changes move fluid within those tubules, which is perceived as a sharp brief pain.
Exposure results from gingival recession, cervical enamel loss, or erosion of the surface.
Why it appears at this age
Recession accumulates over decades, and cervical wear is cumulative. Gum treatment, which is more common in this age group, also exposes root surface that was previously covered.
Reduced salivary flow from medication increases susceptibility, which links this presentation to the broader medication picture in patients over fifty.
The distinction that matters
Dentine sensitivity is sharp and brief, resolving within seconds of removing the stimulus. Pain that lingers for minutes, arises spontaneously, or wakes the patient at night indicates pulpal involvement and requires prompt assessment.
This distinction determines urgency and should be made before any desensitising treatment is undertaken.
Effective management
Dentifrices containing potassium nitrate or stannous fluoride, used consistently over weeks rather than days. Professionally applied fluoride varnish. Sealing of exposed areas with adhesive resin where exposure is localised.
Correction of the causative factor is essential. Where brushing technique or dietary acid is responsible, symptomatic treatment alone produces only temporary benefit.
Establish the cause before treating the symptom. Sensitivity is a signal, and the technique or dietary factor producing it will continue to cause damage if unaddressed.
